Deduction from Salary through Angkasa

This payment method is applicable only for those who work with Government Agencies and is for paying premiums for Life & Group Life policies. To authorise the deduction of premiums or loan repayments from your salary, please complete & sign the Borang BPA1/79 and forward the completed and signed form to the address shown below. The form is available at all MCIS ZURICH offices and our agents.
